We provide a comprehensive care model which is tailored to caring for at-risk seniors with chronic conditions. Our interdisciplinary team of medical, mental health and specialty providers combined with social and whole person care managers work together to develop, monitor and manage patient health daily. We stratify patients based on their conditions, needs and goals, develop personal care plans, and gather patient data daily through remote patient monitoring biometric devices and virtual visits. Guardiant Health’s comprehensive care model uses real-time data and insights to provide care interventions, reducing the need for emergency room visits, hospital stays, and administrative burden while improving care management and access as well as overall patient and provider experience.

We work with health plans to reach, engage, and manage their chronically ill and at-risk populations which require comprehensive care management. Our virtual care model and integrated patient care team manages a members care across the continuum working in concert with primary care and specialty providers, as well as population health and medical management teams. Guardiant’s virtual compressive care management cares for members in their homes providing them with whole person care including medical and mental health, medication and care plan management, social and community services, as well as identification and coordination of in-person in-network services, palliative care, and end-of-life care.
