[MOORPARK, CALIFORNIA, NOVEMBER 21, 2022] Guardiant Health announces a new contract agreement to deploy our Continuous Virtual Care at Home to Regal Medical Group patients post hospitalization. Patients at Risk for possible readmission or emergency care needs will have a dedicated licensed clinical team available 365 days a year and anywhere a patient may live within the Southern California region. Designed for adults who desire the convenience, care, and expertise of a dedicated medical team proactively surveying the needs of members as their health needs or conditions change. Backed by an experienced medical, behavioral, and pharmacy team skilled in reducing the risks and complications of recovery from an acute hospitalization or illness that can result in worsening overall health or disability.

Most seniors who seek care in an office based setting , only see their primary care physician 3-4 times a year on average.
