Vibra Health Plan taps Guardiant Health for virtual 1st care
Guardiant Health is excited to announce its new partnership with Vibra Health Plan!
Reaching people where they are and improving access to care can make the difference in Medicare Advantage members staying healthy at home. As healthcare leaders, we see firsthand the cycle senior patients go through from emergency room visit, to hospital admission, to discharge to skilled nursing facilities, to discharge to home, and then within weeks, the cycle repeats.
Vibra Health Plan, owned by Capital Blue Cross, provides Medicare Advantage benefits to over 6,000 members who reside in rural, urban, and remote areas across the State of Pennsylvania where medical care services may be limited. Reaching, engaging and caring for people who have complex medical, mental and social health needs is foundational to reducing the cost of care, improving member experience and health outcomes. In addition, having a healthcare provider who partners with patients, their caregivers, primary care providers and specialists to help them navigate and manage their care transitions can make a difference in helping them receive the care they need, when and where they need it.
Guardiant Health’s Vibra Care team has been contracted by Vibra and Capital Blue Cross to care for these members across the continuum of care. Guardiant’s virtual first care model, member experience and licensed medical team reaches over 4,000 members, performing health risk assessments, wellness visits and medical visits, gathering proactive insights and delivering bite-sized interventions, virtually and in coordination with the patient’s doctors. Together, we provide our patients with comprehensive care management that includes care delivery to help us reach and care for them on a daily basis, wherever they may be. We focus on what matters most to our patients and help them receive the care they need on an ongoing basis.
