Guardiant Announces Partnership with Imperial Health Plan
We were very excited to receive a call from Dr. Bindru, CEO of Imperial Health Plan, in early December. The Imperial Health Plan (IHP) team asked Guardiant to help attract California, Arizona, and Texas members and work to address and close gaps in care. “Our team came together and began reaching out to IHP members within the week,” states Steve Weiler, Guadiant Health’s Chief Experience and Operations Officer. “Our member engagement and patient care team was amazing in how they reached, scheduled, and conducted virtual health visits for over 500 Medicare members. They demonstrated the power of a built-for-purpose team based care model and clearly exceeded expectations!”

Our “Welcome to Wellness” process moves beyond using the Health Risk Assessment as a compliance only check-the-box activity to proactively connect people with the services they need now, rather than waiting 90 to 120 days or more for retroactive claims and population health data.
Our support of IHP continues to grow as we have outreached close to 1,000 members to date and can see the metrics and impact our digital health model is having on member access to care, satisfaction, retention, and more.
