Guardiant Announces Partnership with River City Medical Group
Guardiant Health is now partnered with River City Medical Group (RCMG), a progressive Independent Physician Association (IPA) uniquely dedicated to serving both the Medi-Cal and Medicare populations, and working to meet their diverse cultural and socio-economic needs.
Guardiant Health serves as an extension of RCMG’s provider network where we are engaging with patients across the continuum of care from “Welcome to Wellness to Care.” Guardiant’s digital health model makes them uniquely positioned to reach patients where they are, complete health risk assessments, as well as clinical wellness visits helping RCMG become a top Medicare Advantage provider while quickly reaching, screening, and caring for patients in the comfort of their home.
In engaging and screening hundreds of patient’s quickly, Guardiant helps to triage and reconnect patients to the medical, mental, and social services they need. This may range from helping them get prescription refills to scheduling much needed appointments with their primary care or in-network special provider, to proactively identifying patients with rising risks. We find that Annual Wellness Visits (AWVs) are hard to complete in a standard primary care or clinic visit as patients often have more urgent needs when they go through the effort to schedule and come into the office, leaving little time to conduct the required AWV.
